Job summary

Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager to join our nursing team delivering compassionate end-of-life care in the Dayton community.

You will empower patients and families, coordinate services, and work with the Clinical Director to ensure high-quality, person-centered care. Schedule is Monday to Friday, 8 am to 4:30 pm; a strong team and onboarding support are provided.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Computer proficiency with typing; experience with EMR systems.
• CPR certification current if required by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Respond to referrals with care and expertise, addressing inquiries compassionately.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct hospice nursing care and coordinate services to support comfort.
• Deliver nursing care rooted in evidence-based practices to ease pain and distress.
• Educate and advocate for patients and families in creating care plans.
• Share call duties as part of a compassionate team commitment.
