Job summary

Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager to join our nursing team. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through the journey with compassion and clinical excellence.

As part of an interdisciplinary team, you’ll empower patients, educate families, and participate in on-call duties. The role requires RN licensure, 1 year of acute care experience, and proficiency with EMR systems.

Qualifications

Hold current unencumbered license as a Registered Nurse.
Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
Must be computer proficient in typing and various programs, including background in EMR.
Possess and maintains current CPR certification if required by state.
Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

Empower Through Engagement: Respond to clinical referral information with care and expertise.
Collaborative Care Champion: Partner with Clinical Director to assess eligibility for hospice services.
Nurturing Direct Care: Deliver hands-on nursing care to ensure comfort and quality of life.
