Job summary

The Care Team Hospice is seeking a compassionate Hospice RN Case Manager to join our team in Cleveland area. This role focuses on delivering expert end-of-life care while building strong relationships with patients and families.

Responsibilities include comprehensive assessments, symptom management, timely visits, and coordination with physicians and the interdisciplinary team to ensure high-quality, patient-centered outcomes.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete scheduled visits, admissions, and clinical interventions with proper documentation in HCHB.
• Respond to patient, family, and caregiver needs, including on-call situations.
• Collaborate with physicians and interdisciplinary team to coordinate care.
• Educate patients and families on symptom management and end-of-life care.
