Job summary

Crossroads Hospice in Dayton, OH is seeking a Registered Nurse Case Manager to provide direct care to patients in homes and care facilities, building trusting relationships with patients and families as the primary link within the interdisciplinary team. The schedule is Monday to Friday, 8:30 AM to 5:00 PM with no mandatory on-call.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and Ability in providing patient care in homes and facilities.

Responsibilities

• Provide direct care to patients in homes or care facilities.
• Listen to patients' needs and respect their wishes.
• Serve as the primary communication link within the interdisciplinary team.
