Job summary

Socket.dev is seeking a Hospice RN Case Manager to coordinate care for hospice patients, deliver skilled nursing services, and implement individualized care plans in partnership with an interdisciplinary team.

The role requires active RN license, 2+ years in hospice, strong assessment, communication, and independent work ethic; valid driver license and CPR certification are essential; EMR experience preferred. This is a full-time weekday position with home visits and EMR documentation.

Qualifications

• Current RN license in state of practice.
• Minimum of 2 years RN hospice experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Valid driver's license and reliable transportation for home visits.
• Current CPR certification.
• Knowledge of regulatory guidelines governing hospice care and documentation.
• Experience with EMR systems preferred.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care including pain and symptom management.
• Coordinate and supervise hospice aides and other healthcare professionals.
• Communicate with patients, families, and the care team about progress and plans.
• Monitor patient wellbeing and comfort during end-of-life care.
• Collaborate with the team to align care with patient goals.
• Ensure regulatory compliance and thorough documentation.
• Provide bereavement support to families and caregivers.
