Job summary

healthcoga is seeking a Hospice Registered Nurse (RN) to plan, organize and direct hospice care for patients in home and community settings. The RN applies nursing expertise to assess needs, develop care plans, educate families and coordinate services with the care team.

The ideal candidate holds an active RN license, CPR certification, a valid driver's license with insured vehicle, and at least two years in public health, home care or hospice nursing.

Qualifications

• Active RN license in the state.
• Current CPR certification.
• Minimum two years in public health, home care, or hospice nursing preferred.
• Valid driver’s license with insured vehicle.
• Ability to work with minimal supervision.

Responsibilities

• Completes comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Develops and initiates plan of care.
• Provides nursing care and administers medications as prescribed.
• Counsels patient and family to address nursing and related needs.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Participates in on-call duties as defined by policy.
