Job summary

Adoration Health in Indiana is seeking a dedicated RN Case Manager for hospice care. The role focuses on 1:1 patient care, coordinating pain control, and implementing individualized care plans in a home-based setting.

The position involves comprehensive nursing duties, collaboration with an interdisciplinary team, and on-call responsibilities within the Carmel service area. Strong communication and documentation are essential.

Qualifications

• Graduate of an accredited nursing program with current RN licensure in Indiana.
• 1 year nursing experience required; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of hospice philosophy of care.
• Must possess CPR certification and be a licensed driver with insured vehicle.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient goals.
• Direct nursing care: administer medications and treatments.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
