Job summary

Adoration Health, an affiliate of BrightSpring Health Services, is seeking a compassionate Hospice RN Case Manager to join our team in the Carmel service area. The role emphasizes patient-centered care, interdisciplinary collaboration, and thorough documentation in a full-time schedule, typically 8 AM–5 PM, Monday–Friday.

You will assess patients’ needs, develop individualized care plans, and educate families while providing direct nursing care and symptom management in home settings.

Qualifications

• Graduate of an accredited nursing program with current licensure to practice in state of operation.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice or similar setting experience preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must have a valid driver’s license with insured vehicle.
• Possess and maintain current CPR certification where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patients’ wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
