Job summary

The Visiting Nurse Association of Wisconsin, Inc. is seeking a Registered Nurse to coordinate end-of-life care for hospice patients and families, with contacts at least every 15 days. You will educate families, review caseloads, and participate in care planning and documentation.

You will lead the interdisciplinary group, supervise aides, and ensure discharge planning and resource coordination, with RN licensure and BLS required.

Qualifications

• RN license issued by the state where the team member practices.
• BLS for Healthcare Providers certification issued by the American Heart Association.
• 1–2 years of nursing experience in home health, hospice, medical/surgical, or oncology.
• Ability to lift and transfer patients safely.

Responsibilities

• Coordinate patient and family care to support terminally ill patients in hospice.
• Educate patients and families on disease progression, hospice care, and symptom changes.
• Lead the IDG to develop and update the Plan of Care and coordinate services.
• Document in the Electronic Health Record per agency standards.
• Supervise Home Health Aide and Licensed Practical Nurse visits as required.
