Job summary

Advocate Health is seeking a compassionate Registered Nurse Case Manager to coordinate hospice care for terminally ill patients and families in Wisconsin. You will lead the IDG, develop the Plan of Care, and ensure seamless service delivery across the care continuum.

Responsibilities include educating families, updating physicians, documenting in the EHR, and guiding discharge planning. A state RN license and BLS are required, with 1–2 years in related nursing settings for eligibility.

Qualifications

• RN license issued by the state where the team member practices.
• Basic Life Support (BLS) certification from the American Heart Association.
• Valid driver’s license.
• 1–2 years of nursing experience in home health, hospice, medical/surgical, LTC, emergency/ICU or oncology.

Responsibilities

• Coordinate patient and family care for terminally ill patients across hospice settings.
• Educate patients and families about disease processes, trajectory, hospice philosophy and end-of-life signs.
• Lead the interdisciplinary care group (IDG) and develop/update the patient's Plan of Care; coordinate services.
• Document in the electronic health record per agency/department standards and policies.
• Oversee discharge planning and mentor paraprofessional staff as needed.
