Job summary

GHPCO in Buford, GA is seeking a Registered Nurse to provide compassionate hospice nursing care to patients at home or in facilities. You will assess needs, implement care plans, and coordinate with the interdisciplinary team to support patients and families during end‑of‑life care.

The role requires timely documentation, supervision of aides, medication administration, and close collaboration with physicians to control pain and symptoms.

Qualifications

• A Registered Nurse licensed to practice in Georgia with at least one year of hospice or home health experience.
• Valid driver’s license and access to a personal vehicle for on-the-job transportation.
• Ability to develop and implement improved methods of operation.
• Ability to supervise objectively and evaluate strengths and weaknesses of team members.
• Ability to solve practical problems and interpret instructions.
• Excellent verbal and written communication skills.
• Visual acuity to operate a computer terminal and read data.
• Must meet visual requirements for driving.

Responsibilities

• Assess home care and physical needs of the patient.
• Provide direct nurse care to patients as designated in the care plan.
• Assume primary responsibility of care management.
• Document visit records electronically within 24 hours of the visit.
• Make supervisory visits to assess home health aide performance at least every two weeks.
• Communicate with the primary physician regarding patient status and pain management.
• Document all care in the patient’s chart per hospice requirements.
• Attend patient care conferences to coordinate care with other providers.
• Administer medications and treatments per orders and applicable laws.
• Teach and counsel patients and families on caregiving techniques.
• Supervise the home health aide plan of care.
• Coordinate ongoing patient care with the interdisciplinary team.
• Ensure effective use of resources and confidentiality.
• Participate in quality improvement programs and hospice initiatives.
