Job summary

GHPCO in Georgia seeks a Registered Nurse to provide compassionate hospice care, assess patient needs, and coordinate with the interdisciplinary team to develop and implement care plans. You will document visits, supervise home health aides, and educate patients and families on care techniques, ensuring compliance with physician orders and organizational policies.

Travel by car is required; on-call weekends/evenings may be part of the schedule.

Qualifications

• RN license to practice in the state with hospice/home health experience.
• One year of hospice or home health experience preferred.
• Driver’s license and vehicle for on‑the‑job travel.
• Ability to develop and implement improved methods of operation.
• Strong verbal and written communication skills.
• Visual acuity to operate vehicles and equipment.

Responsibilities

• Assess home care and physical needs of the patient.
• Provide direct nurse care as per care plan.
• Assume primary care management responsibilities.
• Document visit within 24 hours electronically.
• Supervise home health aide visits every two weeks.
• Coordinate with primary physician on status and pain control.
• Document care in patient records per guidelines.
• Attend care conferences to coordinate with providers.
• Administer medications per orders and standards.
• Educate patients and families on caregiving techniques.
• Coordinate ongoing care with the interdisciplinary team.
• Ensure confidentiality and compliant documentation.
