Responsibilities

• Provide professional nursing care for hospice patients in homecare and facility settings.
• Perform initial and ongoing assessments of patients and update plans of care.
• Coordinate patient care including medications, DME, and referrals with the IDT.
• Educate patients, families, and facility staff on hospice philosophy, symptom management, and care coordination.
• Supervise CNAs, LVNs, and homemakers and maintain professional boundaries.
• Ensure timely documentation in agency and facility records and coordinate transitions of care.
