Job summary

Hospice Austin is seeking an RN Case Manager to provide comprehensive nursing care for patients in homecare and assigned facilities in the Spicewood/Austin area. You will conduct admissions visits, assess patient needs, and coordinate the plan of care across the interdisciplinary team.

The role includes supervising CNAs/LVNs and educating facility staff on hospice philosophy, symptom management, and care coordination processes. Valid Texas RN license and CPR are required.

Qualifications

• Graduate of an approved school of professional nursing.
• Unrestricted RN license by the Texas Board of Nurse Examiners.
• Valid Texas driver's license, auto liability insurance and safe driving record.
• CPR certification.

Responsibilities

• Provides professional nursing care for hospice patients in the homecare and assigned facility settings.
• Performs initial and ongoing assessment of patients' physical, psychosocial, and environmental needs.
• Develops and updates individualized plans of care and coordinates transitions of care.
• Collaborates with physicians and IDT to ensure high-quality hospice care.
• Supervises CNAs, LVNs, and homemakers, and documents care activities.

Education

Registered Nurse license (Texas).
