AccentCare, Inc. is seeking a Hospice Registered Nurse (RN) Case Manager in Texas to join our interdisciplinary team in providing compassionate, expert care for hospice patients and families.

Qualifications

• RN license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide individualized nursing care maximizing comfort and quality of life.
• Partner with patients, families, physicians, and interdisciplinary team to carry out care plans.
• Perform comprehensive assessments—physical, emotional, social, and spiritual to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly reflecting patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
