Job summary

Gentiva seeks an experienced Hospice RN / RN Case Manager to lead in home-based end-of-life care. You will assess patients, deliver skilled nursing care, and coordinate with an interdisciplinary team. You’ll educate families, manage symptoms, and document care plans across settings.

Ideal candidates have at least 1 year as an RN, hospice or palliative care experience preferred, and hold a current state license with CPR. Valid driver’s license and insurance required.

Qualifications

• Current license to practice nursing in the state of employment.
• CPR certification.
• Valid driver’s license and automobile insurance.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments with education for patients, families, and caregivers.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update plans of care, including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and with physicians.
• Educate patients and caregivers on disease process, self-care, medications, and end-of-life needs.
• Document timely clinical records and communicate condition changes to the care team.
• Support continuity of care and transitions between levels of care.
