Qualifications

• RN license in the state of employment
• Hospice or palliative care experience preferred
• CPR certification required

Responsibilities

• Perform comprehensive patient assessments and reassessments, education to patients and caregivers
• Deliver skilled nursing interventions and symptom management per care plans
• Develop and update individualized plans of care and visit schedules
• Coordinate care with interdisciplinary team including physicians, LNPS, CNAs, social workers, and chaplains
• Provide education on disease processes, end-of-life care, medications, nutrition, and self-care
• Document timely clinical records and maintain infection control standards
• Support IDG participation and on-call patient support activities
