Job summary

Advocate Health is seeking an experienced Hospice Registered Nurse to perform comprehensive nursing assessments, triage, and interventions for hospice patients during after-hours shifts. Qualifications

• Completion of an accredited or approved nursing program.
• Active RN license in the state of practice.
• BLS certification – American Heart Association.
• Valid driver’s license.

Responsibilities

• Provide hospice nursing assessment, triage, and intervention during after-hours shifts.
• Respond to patient and caregiver calls to assess symptoms and educate on interventions.
• Conduct in-person visits to manage acute symptom changes and provide crisis support.
• Document all encounters and communications accurately in the EMR.
• Collaborate with physicians and the care team to coordinate changes in patient condition.
