Qualifications

• RN with Bachelor of Science in Nursing required.

Responsibilities

• Develop individualized care plans based on assessments, patient wishes, and directives.
• Collaborate with interdisciplinary teams to ensure comprehensive care.
• Administer medications and treatments to manage pain and distressing symptoms.
• Monitor and evaluate the effectiveness of interventions to ensure comfort.
• Provide emotional support to patients and families, addressing fears and distress.
• Facilitate conversations about feelings, wishes, and end-of-life goals.

• Maintain accurate electronic health records reflecting assessments and outcomes.
• Ensure adherence to ethical guidelines and legal regulations in end-of-life care.
