Job summary

VIA Health Partners is seeking compassionate CNAs or equivalent to provide in-home patient care for terminally ill and geriatric patients in the Clemson area. You will perform essential ADLs, monitor vital signs, assist with meals, and support safety and comfort in the patient’s home environment.

Minimum 1 year of experience required; home care experience preferred. The role emphasizes teamwork, compassionate patient care, and adherence to care plans and safety protocols.

Qualifications

• Minimum 1 year experience required.
• Experience in home care, working with terminally ill and/or geriatrics preferred.

Responsibilities

• Delivery of patient care including ADL such as bathing, bed bath, tub bath, shower, sitz.
• Assist with body mechanics, transfers, ambulation, range of motion.
• Nutritional support: assist with meals, measure intake/output.
• Assist with elimination including bedpan/urinal care and catheter care.
• Maintain safety: infection control, CPR/Heimlich, use of restraints where applicable.
• Perform postmortem care, vital signs, wound care and dressing changes as needed.
• Housekeeping to ensure health and safety of patient/family/caregiver.
• Participate in interdisciplinary team conferences (IDT).
• Sterile dressing/wound care, oxygen therapy setup and monitoring.
