Job summary

VIA Health Partners in the community is seeking a compassionate Nursing Assistant to deliver patient care in home hospice settings. Responsibilities include ADLs, vital signs, basic nursing tasks, and collaborating with an interdisciplinary team to support patients and families.

Qualifications

• Minimum 1 year experience required.
• Experience in home care, working with terminally ill and/or geriatrics preferred.

Responsibilities

• Delivery of patient care including ADLs such as bathing, mobility assistance, nutrition support, elimination and safety measures.
• Participates in interdisciplinary team conferences and ensures accurate clinical documentation.
