Job summary

VIA Health Partners is a nonprofit hospice and palliative care provider seeking a Certified Nursing Assistant (CNA) to deliver compassionate patient care in home-based settings in Fort Mill, SC. Responsibilities include ADL support, vital signs, basic nursing tasks, and collaborating with the interdisciplinary team to meet patient needs.

• High School diploma preferred, plus training/certification in related field required.
• Certified Nursing Assistant; maintain current listing status required.
• NA – must be in good standing with NC Department of Health and Human Services
• Minimum 1 year experience required.

Responsibilities

• Delivery of patient care.
• Provides personal care (ADL) including bathing, mobility, nutrition, elimination, safety and vital signs.
• Performs housekeeping and ensures a clean, safe patient environment.
• Participates in interdisciplinary team conferences and communicates patient needs.
