VIA Health Partners is hiring a Certified Nursing Assistant to provide direct patient care in home-based hospice settings. The role includes ADL assistance, meal support, vital signs, documentation, and collaboration with the care team to ensure comfort and safety for patients and families.

Requirements include CNA certification, current licensure, and at least 1 year in home health or geriatric care. Competitive benefits are provided and orientation is thorough.

Competenze

• Certified Nursing Assistant; maintain current listing status required.
• Minimum 1 year experience required. Home care or geriatrics preferred.
• CPR certification preferred or maintained per policy.

Mansioni

• Deliver patient care as an NA, including ADL support (bathing, grooming, mobility).
• Assist with meals, nutrition, intake/output, and feeding support.
• Monitor, document vital signs and patient status; ensure safety and infection control.
• Assist with housekeeping, room cleanliness, and supply stocking.
• Participate in interdisciplinary team conferences (IDT) and referrals as needed.
• Document accurately and timely per HPCCR guidelines.
• Provide postmortem care as assigned and support with crisis response.
• If applicable, support with wound care, catheter care, and oxygen setup per protocol.
