VIA Health Partners is seeking a compassionate Certified Nursing Assistant for home-based hospice/palliative care in the Huntersville area. You will provide direct patient care, assist with daily living activities, monitor health status, and support families.

The role involves bedside care, documentation, and collaboration with the care team. Requirements include active CNA certification, NC listing, and at least 1 year in home care; hospice experience is preferred.

Competenze

• Certified Nursing Assistant; maintain current listing status required.
• Minimum 1 year experience in home care, preferably with terminally ill or geriatrics.
• High School diploma preferred, plus related field training.
• Experience in hospice or palliative care is a plus.

Mansioni

• Provide personal care (ADL) including bathing, transferring and assistance with activities of daily living.
• Take vital signs and monitor patient condition as directed.
• Assist with mobility, nutrition and hydration, and oxygen therapy as required.
• Participate in interdisciplinary team conferences and maintain accurate documentation.
• Perform household tasks and ensure clean, safe patient environments.
