Job summary

Heartland Hospice in Myrtle Beach, SC is seeking an experienced Registered Nurse to join our hospice team as a dedicated home-based case manager. You will assess, plan, and deliver skilled nursing care with a compassionate approach that puts patients and families first.

You will collaborate with physicians, social workers, and other professionals to create personalized care plans, educate families, and coordinate symptom management.

Qualifications

• RN license in the state of employment.
• Minimum 1 year RN experience in hospice, palliative care, home health, or related settings.
• CPR Certification is required.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Be the primary contact for patients and families throughout the hospice journey.
• Deliver and document skilled nursing care per the Plan of Care in patients' homes or other settings.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with an interdisciplinary team to refine personalized care plans.
• Provide education, support, and bereavement guidance to patients and families.
• Teach caregivers how to care for loved ones safely and confidently.
• Communicate updates on patient condition to the care team and coordinate interventions.
• Maintain accurate clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
• Navigate hospice concepts, symptom management, and end-of-life processes with patients and families.
