PruittHealth is seeking a Hospice Case Manager for a full-time, on-site role in Myrtle Beach, SC. You will coordinate and deliver comprehensive hospice care, manage caseloads, monitor symptoms, educate patients and families, and document care per regulatory standards.

Collaborating with physicians and a multidisciplinary team, you will advocate for patient goals, ensure person-centered care, and travel locally to patient residences as needed. RN licensure in SC is required.

Qualifikationen

• Clinical hospice care skills including patient assessment, care planning and symptom management.
• Strong case management abilities including caseload coordination and care transitions.
• Effective communication and interpersonal skills with patients, families, and professionals.
• Organizational and documentation skills to maintain accurate records and comply with regulations.
• Active RN license in South Carolina; hospice or palliative care certification preferred.
