Gentiva Hospice is seeking a compassionate Registered Nurse to serve as the RN Case Manager in the Spartanburg area. You will perform comprehensive patient assessments, develop plans of care, and coordinate with an interdisciplinary team in patients' homes.

This role emphasizes education, symptom management, and bereavement support for patients and families, with clinical documentation, on-call coverage, and a commitment to quality care in end-of-life settings.

Qualifications

• RN with licensure in the requested employment state
• Minimum of one year experience as an RN in hospice, palliative care, or related settings
• Hospice or palliative care experience preferred

Responsabilités

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments
• Deliver skilled nursing interventions and symptom management per care plans
• Develop and update individualized plans of care and visit frequency
• Coordinate care across interdisciplinary teams and communicate with physicians and other providers
• Educate patients and caregivers on disease, self-care, end-of-life care, and medications
• Document timely clinical records and ensure regulatory compliance
• Support on-call and patient support activities as applicable
