You will provide patient and family psychosocial support, assess needs, collaborate with the interdisciplinary group, and assist with discharge planning and community resources.

The role requires an MSW and GA licensure, with experience in healthcare and a commitment to compassionate end-of-life care.

Competenze

• Master's or Doctorate in social work from an accredited school.
• At least two years of experience in a health care setting.
• MSW licensure requirements and valid license to practice in Georgia.
• Registration to practice as a Medical Social Worker if applicable.
• Ability to meet health clearance and references requirements.
• Willingness to travel to patient homes or facilities.

Mansioni

• Assumes primary responsibility for medical social services for hospice patients and families.
• Assesses social and emotional factors to estimate capacity to cope with terminal illness.
• Collaborates with Interdisciplinary Group to develop care plans within two days.
• Provides psychosocial support and bereavement counseling as needed.
• Documents observations and interventions accurately.
• Participates in discharge planning and community resource referrals.
• Supervises counseling activity and coordinates volunteers.
• Promotes awareness of hospice care in the community.
