Job summary

UnitedHealth Group is seeking a Medical Social Worker to provide psychosocial support, assessment, and care coordination for home-based patients and families. You will collaborate with an interdisciplinary team to connect patients with community resources and support safe transitions.

The role requires a Master’s and/or Doctorate in Social Work, active LMSW or LCSW licensure, CPR certification, and at least one year of health care social work experience.

Qualifications

• Masters and/or Doctorate degree in Social Work from an accredited school
• Active licensure in LMSW or LCSW
• Current CPR certification

Responsibilities

• Assists patients and families with health-related financial, social and emotional concerns and helps with coping skills
• Provides social work services in accordance with the plan of care
• Communicates changes in clinical status to the healthcare team and care center staff
• Educates patients and families and other team members on social work interventions and treatment plans
• Performs care planning, transfer and discharge functions
• Identifies risks in the patient’s environment and coordinates interventions with the physician and team
• Other duties as assigned
