Job summary

Preferred Care Home Health Services is seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our growing home health team. As an LPN in home health, you’ll provide skilled nursing care in patients’ homes under the direction of a Registered Nurse, with emphasis on patient relationships, clinical quality, independence, and teamwork.

Qualifications

• Florida LPN license is required.
• Graduate of an approved practical nursing program.
• Current CPR certification is required.
• Previous home health experience is preferred.
• Valid driver’s license with reliable transportation.
• Strong clinical assessment, communication, organization, and documentation skills.
• Ability to work independently while collaborating with an interdisciplinary care team.
• Compassionate, professional, patient-centered approach to care.

Responsibilities

• Provide skilled nursing services in accordance with the physician’s plan of care and home health certification.
• Provide nursing care under the direction and supervision of a Registered Nurse.
• Administer medications and treatments as ordered.
• Monitor patients for changes in condition and communicate findings to supervisor.
• Document patient care, clinical findings, treatments, and interventions accurately.
• Prepare timely clinical notes in accordance with agency requirements.
• Participate in case conferences, reviews, and ongoing education.
• Assist with developing and evaluating the patient’s plan of care with the interdisciplinary team.
• Educate patients and caregivers about medications, treatments, and disease processes.
