Job summary

3HC Home Health & Hospice Care Inc. is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to home health and hospice patients as outlined in each patient’s plan of care.

You will support the RN, document care, and collaborate with the team to ensure compassionate, high-quality service with a focus on patient comfort and dignity. Requirements include NC Licensure as an LPN, CPR certification, and reliable transportation.

Qualifications

• Licensed Practical Nurse licensed to practice in North Carolina.
• Minimum of two years of experience as a nurse in an acute care setting preferred.
• Maintain current CPR certification (BCLS).
• Must be dependable and capable of dealing pleasantly and tactfully with patients, family, staff, providers, and the public.
• Possess valid Driver's License and reliable transportation.

Responsibilities

• Administer skilled nursing care to home health and hospice patients as outlined in the individualized plan of care.
• Observe and report signs and symptoms, reinforce RN teaching, and assist with delegated tasks as appropriate.
• Document in the medical record electronically and coordinate with RN and physician as needed.
• Collaborate with supervisor to meet agency clinical needs and participate in on-call coverage as required.
• Maintain communication with supervisor and participate in meetings and quality improvement activities.
