The Care Team is seeking a dedicated Hospice Licensed Practical Nurse (LPN) to deliver compassionate, hands-on care to patients and families in home and facility settings. You will work under RN supervision to support comfort, symptom management, and dignity at end-of-life, helping the interdisciplinary team provide excellent care.

This role requires an active LPN license, driving ability, and a commitment to the mission-driven hospice environment.

Qualifications

• Active LPN license required.
• Hospice or long-term care experience preferred.

Responsibilities

• Provide nursing care as delegated by the RN.
• Monitor patient status and report changes.
• Support comfort and symptom management.
• Document care provided in the EMR.
• Collaborate with the interdisciplinary team.
