Job summary

Professional Healthcare Resources is seeking a dynamic Licensed Practical Nurse (LPN) to join our team of clinicians. The LPN will support home health care and hospice patients under the direction of an RN Field Case Manager, following the developed plan of care.

Responsibilities include carrying out treatments and assessments, maintaining records, and providing daily reports to the Clinical and Case Managers.

Qualifications

• Graduate of practical nursing program and licensed in state(s) of operation.
• At least 1 year of clinical experience.
• Experience in community or home health preferred.

Responsibilities

• Carry out treatments, teaching, and observation per Plan of Care and delegated by the case manager.
• Maintain records and reports per regulatory requirements and company policies and procedures.
• Provide daily report to Clinical Manager and Case Manager to review status of cases.
• Participate in personal and professional growth and development.
• Fulfill additional performance responsibilities as assigned and as necessary.
