Job summary

Assured Hospice, part of LHC Group within the Optum family of organizations, is seeking a Licensed Practical Nurse (LPN) to provide compassionate hospice care in patients' homes. You will work under a staff RN, administer medications, assist with daily activities, and support families in navigating complex health needs.

Required: valid LPN license, valid driver's license with access to a reliable vehicle, and CPR certification (or ability to obtain within 90 days).

Qualifications

• Current and unrestricted LPN licensure in state of practice
• Current driver's license and vehicle insurance, and access to a dependable vehicle or public transportation
• Current CPR certification or ability to complete within 90 days of hire
• Hospice experience

Responsibilities

• Performs skilled nursing services under the supervision of a staff RN daily
• Administers prescribed medications and treatments as permitted by state regulations
• Assists the physician and/or staff RN in performing specialized procedures as needed
• Assists the patient with daily activities and encourages appropriate self-care daily
• Reports to the patient's physician and other appropriate personnel regarding pertinent changes in the patient's condition as needed
• Performs complex wound care if in-service is documented for specific procedure daily or as needed
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
