Job summary

Qualifications

Responsibilities

• Provide direct patient care to hospice patients.
• Complete documentation per policy and regulation.
• Follow care plans developed by the RN.
• Coordinate hospice services with the RN and care team.
• Transcribe physician orders and forward notes to the RN.
• Attend and participate in IDT meetings.
