Job summary

Goodwin Living in Alexandria, VA seeks a full-time Licensed Practical Nurse to provide end-of-life and hospice care in the community setting under the direction of the RN Case Manager. The LPN will support planning for patient and family care and deliver skilled nursing as outlined in the care plan.

Qualifications

• Graduate of an accredited school of practical nursing.
• Current Commonwealth of Virginia LPN license.
• Two years recent nursing experience in acute, long-term care or home care/hospice, with at least one year in hospice or homecare preferred.

Responsibilities

• Provide nursing care, including physical and psychological care, per policy and nursing standards.
• Consult with RN Case Manager for changes in care plan.
• Recognize and report significant changes in patient condition to RNCM.
• Administer medications and treatments as ordered by physician, excluding IV push.
• Reinforce interventions, medications, and side effects to patient and family.
• Participate in interdisciplinary group meetings and share professional support.
