Job summary

PeaceHealth is seeking a Home Health Aide - Hospice to join our team in a full-time, day position. The role provides direct patient care and light nursing support under clinical supervision in a home health/hospice setting.

Responsibilities include assisting with activities of daily living, documenting care per regulatory guidelines, and coordinating with the RN case manager to ensure smooth, quality care for adult and possibly pediatric patients as needed.

Qualifications

• Minimum 2 years CNA or Home Health Aide experience in care settings.
• Must have Oregon CNA certification.
• Valid driver's license and auto insurance.
• Basic Life Support required upon hire.

Responsibilities

• Perform patient care tasks per care plan.
• Take vital signs and assist with medication as directed by nurse.
• Assist with personal care and activities of daily living.
• Document accurately and timely per guidelines.
• Support walking, transfers, and mobility; assist with equipment.
• Provide light housekeeping and meal support as outlined in care plan.
