Qualifications

• Basic Life Support certification on hire.

Responsibilities

• Perform patient care per care plan.
• Take and record vital signs; assist with medications as directed.
• Assist with personal care and activities of daily living.
• Support walking, movement, and use of assistive devices.
• Prepare meals, assist with feeding, light housekeeping per care plan.
• Document care accurately and timely per guidelines.
• Communicate health changes to RN/therapist case manager.
