Job summary

Advocate Health is seeking a Hospice Home Health Aide to provide personal care to end-of-life patients within the Advocate Aurora Hospice division. The role supports the RN’s plan of care and requires compassionate, patient-centered care in a home health setting.

Regular hours are 8:00 AM to 4:30 PM with on-call weekend rotation and holiday coverage. The position covers the Chicago area including Southeast Side communities; travel within the service area is expected.

Qualifications

• Must be certified as a nurse assistant in the state in which the teammate practices.
• Nurse assistant in good standing with the State Nurse Aide Registry with eligibility to work in home health agencies and hospices.
• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months.
• A valid driver’s license issued by the Division of Motor Vehicles.

Responsibilities

• Contributes to effective patient outcomes. Provide competent patient care assigned by RN and plan of care.
• Performs personal care needs including feeding patient, laundry, light cleaning of patient room and washing; monitors safety and uses equipment properly.
• Takes vitals as directed by the RN and assists with patient positioning and comfort measures.
• Maintains HIPAA/privacy; documents care and communicates changes in patient condition to RN or supervisor.
