Advocate Health at Home seeks a Chaplain to provide holistic spiritual care to patients, families, and staff in home-based hospice. The role includes counseling, crisis response, sacramental support, and collaboration with interdisciplinary teams to ensure compassionate, comprehensive care.

The chaplain will travel within the Advocate Aurora service area, participate in on‑call rotation, and support patients and families from diverse backgrounds during end‑of‑life journeys.

Competenze

• At least two CPE units accredited by ACPE; NACC units may be considered.
• Five units preferred.
• In good standing with the appropriate faith authority.
• A valid driver’s license issued by the DMV.
• Completed masters degree (MA, MDiv) in religious studies, theology, divinity or related field.
• Strong background in functional theology and pastoral application to individuals/families.
• Excellent communication skills with knowledge of counseling techniques.
• Ability to identify clients’ needs with consideration to diverse backgrounds.
• Experience and compassion in ministering to those who are dying and their grieving families.

• Participates in the QA/CQI process for Advocate Hospice.
