Advocate Health is hiring a Home Health Aide for the Green Bay team. The role supports patients in the Green Bay market with occasional travel to nearby territories. The ideal candidate is compassionate, reliable, and able to follow care plans under RN supervision.

Position involves 40 hours per week, Monday–Friday, 8:00 a.m.–4:30 p.m. and requires CNA certification, BLS within 6 months, and a valid driver's license. Company offers comprehensive benefits and training opportunities.

Qualifications

• Willingness to travel within the Green Bay market as needed.

Responsabilités

• Provide competent patient care under RN guidance and plan of care.
• Take temperatures, pulse, respiration and blood pressure as directed.
• Support with activities of daily living and hygiene needs.
• Document care provided and communicate changes promptly.
• Maintain patient confidentiality and HIPAA compliance.
• Adhere to safety rules and assist with mobility and repositioning.
