You will assess patients, triage symptoms, educate caregivers, and collaborate with physicians, with EMR documentation and adherence to hospice standards.

Requirements include an active RN license, BLS, one year of hospice or related nursing experience, and a commitment to compassionate end-of-life care.

Qualifications

• Completion of an accredited nursing program.
• Active RN license in the state of practice.
• BLS certification - American Heart Association.
• Typically one year of nursing experience in any clinical setting.
• Willingness to travel to patient homes in Milwaukee and surrounding areas.

Responsibilities

• Provide after-hours hospice nursing assessments and triage.
• Respond to calls to assess symptoms and educate caregivers.
• Conduct in-person visits to manage acute symptoms and ensure comfort.
• Document encounters accurately in the EMR.
