Job summary

GBMC HealthCare is seeking a Hospice Home Care Nurse in Maryland to provide compassionate nursing and supportive care to terminally ill patients and their families in a home or facility setting.

You will perform patient assessments, develop and implement care plans with the interdisciplinary team, and educate families on symptom management, medications, and safety. A Maryland RN license and ADN/BSN are required; two years of nursing experience preferred.

Qualifications

• Two years of nursing experience or equivalent education and experience.
• Diploma, ADN, or BSN from an accredited program.
• Current Maryland RN license.
• Knowledge of universal precautions and infection control.
• Excellent communication and interpersonal skills.

Responsibilities

• Completes initial assessment of patient and family to ascertain home care needs.
• Prepares and implements the care plan with the Interdisciplinary Group.
• Provides direct nursing care and coordinates with the primary physician.
• Plans and supervises home health aide care.
• Updates physician and interdisciplinary team during care conferences.
• Teaches family and facility staff patient care skills, including medications.
