Shalom Hospice in Tennessee is seeking aRegistered Nurse to plan, organize, and direct hospice care within patients’ homes and communities. You will lead nursing care using the nursing process and develop care plans with patients and families.

You will assess, revise plans as needed, administer prescribed treatments, counsel families, and coordinate care with the interdisciplinary team, including on-call duties and ongoing education.

Qualifikationen

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• 1 year of recent professional nurse experience or a BSN with 2 years, at least 1 in public health, home care, or hospice.
• Management experience not required; supervise hospice aides.
• Contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving and math skills.
• Prolonged walking/standing; able to lift and transfer patients and equipment.
• Licensed driver with insured vehicle in good working order; CPR certification required.

Aufgaben

• Complete initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Develop and revise plan of care with patient and family participation; write and re-evaluate plan as needed.
• Initiate and revise the plan of care as patient status changes.
• Use health data to determine nursing diagnosis and establish goals in a care plan.
• Counsel patient and family in meeting nursing and related needs; provide health instructions.
• Act as Case Manager when assigned and coordinate patient care for caseload.
• Maintain accurate clinical notes and communicate changes to physicians and team.
• Ensure safe environment and continuity of care with interdisciplinary group.
• Participate in on-call duties and quality improvement activities.
