VITAS Healthcare is seeking a Home Health Aide (HHA) to provide essential personal care and support to hospice patients in their homes. The HHA will work under the direction of a VITAS Nurse, following a written plan of care, and will contribute to maintaining patient safety, comfort, and quality of life.

Responsibilities include assisting with hygiene, meals, mobility, and monitoring patient status while documenting observations for the clinical team.

Qualifications

• Reading and writing abilities to document patient status.
• Must be able to travel to patient homes as required by assignments.
• Ability to communicate effectively with patients, families, and clinicians.

Responsabilités

• Performs personal care activities as outlined in a written assignment.
• Maintains patient hygiene and assists with meals and feeding as needed.
• Assists with ambulation and basic mobility aids.
• Assists with prescribed treatments under nurse supervision and documents observations.
• Records temperature, pulse and respiration and reports changes to the nurse.
