Job summary

Choice Health at Home is seeking a Hospice Field RN to provide compassionate, patient-centered nursing in home and hospice settings in Grapevine, TX. You will assess, coordinate care, and collaborate with physicians, social workers, and chaplains to support patients and families facing life-limiting illnesses.

You will perform on-call duties, ensure care plan implementation, and educate families on end-of-life care while upholding dignity and comfort for patients.

Qualifications

• Current RN license
• CPR certification
• Valid driver's license
• Valid auto insurance
• Hospice RN experience preferred
• HomeCare HomeBase experience preferred

Responsibilities

• Initial and ongoing comprehensive assessments of patients in home settings to address physical, functional, psychosocial, and environmental needs.
• Apply admission and recertification criteria to establish appropriate levels of care.
• Develop and document the plan of care to ensure quality and continuity of service.
• Educate patients and families about disease process, end-of-life care, and self-care techniques; provide staff training as needed.
• Determine scope and frequency of services based on acuity and needs; assess caregiver ability.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure quality care.
• Prepare progress notes showing goal advancement and ensure proper documentation.
• Inform physicians of changes and monitor responses; participate in on-call duties.
