Job summary

Bristol Hospice is seeking experienced Certified Home Health Aides (CHHA) who bring compassion and dignity to patient care. Under RN supervision, you will support terminally ill patients in their homes, delivering personal care, meals, and comfort while maintaining strict confidentiality and professional conduct.

You will work as part of an Interdisciplinary Group Team, perform routine assessments, document care, and coordinate with Case Managers to ensure plans of care are followed.

Qualifications

• Must be a Certified Nursing Assistant (CNA) in good standing
• California residents must have a current Home Health Aide (HHA) certification
• Preferred 1 year experience in community health/hospice or medical/surgical settings

Responsibilities

• Provide personal care including baths, back rubs, oral hygiene, and changing bed linens
• Assist in dressing and undressing patients as assigned
• Plan and prepare nutritious meals and assist in feeding
• Take and record vital signs when ordered
• Assist with ambulation, transfers, and range of motion
• Maintain clean and orderly living area and adhere to care procedures
• Document care provided and report changes to Case Manager
• Provide respite for family/caregiver when on-site
• Support safe use of medical equipment and proper safety measures
