Qualifications

• CNA required: current certification and valid license.
• High school level coursework in English and basic math or equivalent.
• Experience in direct patient care in hospitals, nursing facilities, or similar settings preferred.

Responsibilities

• Perform personal care tasks for hospice patients with compassion and efficiency.
• Communicate effectively with patients, families, and Case Manager while maintaining confidentiality.
• Assist with bathing, grooming, feeding, and daily activities as assigned.
• Record vital signs, monitor patient condition, and report changes promptly to the hospice team.
• Support mobility, transfers, and range-of-motion activities as instructed by caregivers.
