Job summary

Gentiva Hospice is seeking a compassionate Hospice Aide/CNA to provide direct, hands-on personal care to patients in home or facility settings. You will support daily living activities, mobility, meals, and comfort, while documenting visits and communicating changes to the care team.

The role emphasizes empathy, resilience, and the ability to work independently in home health, hospice, or long-term care environments.

Qualifications

• Active CNA license in the state of employment (Required).
• Current CPR certification (Required).
• Driver’s license and reliable vehicle with insurance (Required).
• Six months of direct patient care experience (Required).

Responsibilities

• Provide direct personal care to terminally ill patients.
• Assist with personal hygiene, grooming, and toileting.
• Support safe mobility with walkers and wheelchairs.
• Help with meal prep, feeding, and light housekeeping.
• Observe and report changes in patient’s condition.
• Offer emotional support to patients and families.
• Maintain accurate visit documentation.
• Attend team meetings and participate in Quality Improvement activities.
