St. Croix Hospice is seeking a compassionate Hospice Aide (CNA) to join our interdisciplinary team. You will provide direct personal care, assist with mobility, meals, and comfort while following each patient’s Plan of Care under RN supervision.

You will document care activities, report changes to the RN Case Manager, maintain HIPAA compliance, and support families. A valid CNA license, 18+, clean driving record, and reliable transportation are required.

Qualificações

• Must have an active CNA license in the state of practice.
• Must be at least 18 years of age.
• Ability to pass background check, consumer report and drug test.
• Valid State Driver’s License and reliable vehicle required.

Responsabilidades

• Provide personal care including bathing, dressing, oral hygiene and bed linen changes.
• Assist with mobility, transfers and exercises as instructed by nursing staff.
• Assist with meals, feeding and light household tasks as assigned.
• Monitor patient condition and report changes to the RN Case Manager.
• Maintain documentation and HIPAA compliance for all care activities.
• Support families with emotional care and respite as needed.
